Digital August Packet

Welcome to our new digital August Packet!

In an effort to simplify our back-to-school paperwork, we have moved our traditional August Packet to an online format. Below you will find all of the forms and acknowledgments that need to be completed for your family before the start of the school year. All of the other information typically found in the August Packet are located beneath the forms below.

This is a new process for us, and we appreciate your patience as we make this transition. We hope this new system makes completing and submitting your back-to-school forms more convenient. Physical Examination -

All students must have a physical examination completed within the past 12 months. Families may submit the physical examination form provided by their healthcare provider or have their provider complete the school's Physical Examination Form (found at this link). Either form will satisfy this requirement, and need to be turned into the Nurse or Front Office by the start of school.

The following form is required for all students in grades 5th-8th, and must be completed before the start of the school year. Please review this form carefully, and submit all required information.
